Reimbursement Information for US Invited Participants
Receipts and Per Diem
Tokyo, Japan is considered a high cost area by the University of Georgia (UGA). There is no explicit per diem on meals or lodging according to UGA. However, we ask that you retain all receipts for all expenses incurred to avoid problems with customary reimbursement. Please keep in mind that in 2003, when the original grant was written, the federal per diem for Tokyo was $225 for lodging and meals.
Reimbursement Forms
If you are a US Citizen and wish to obtain reimbursement from UGA you will need to fill out the following forms:
* Honoraria, print two forms
--- form (1) should have everything filled out including your signature on line (9)
--- form (2) should only include your signature on line (9)
* USC expense statement, print one form,
sign at the bottom above the word "Signed"
If you are a Nonresident Alien (NRA) and wish to obtain reimbursement from UGA you will need to fill out the following forms:
* Honoraria, print two forms
--- form (1) should have everything filled out including your signature on lines (9) and (11)
--- form (2) should only include your signature on line (9) and again on line (11) where indicated
* NRA expense statement, print one form,
sign at the bottom above the word "Signed"
* NRA checklist, print one form,
use section "C" as a checklist
All forms and original receipts must be mailed, together, to:
